What we have learned about this county

• The south end of the county runs onto the Palmer Divide, and the 18-mile stretch of I-25 from south of Castle Rock to Monument - the section CDOT calls the Gap - is the only through route: no frontage road, no parallel alternate. CDOT's own project record for that stretch tells you what the grade does in winter: it built truck climbing lanes near Monument Hill and the Greenland exit and a new truck chain-up station on southbound I-25 at Larkspur. A Castle Rock or Larkspur patient with a Colorado Springs appointment needs a same-day weather call rather than a fixed pickup time.
• Douglas County has four acute-care hospitals - Sky Ridge in Lone Tree, UCHealth Highlands Ranch, AdventHealth Parker and AdventHealth Castle Rock - plus Highlands Behavioral Health System in Highlands Ranch, which is psychiatric and has no emergency department. Not one of them is a Level I trauma centre; Sky Ridge is Level II. Every patient who needs Level I care leaves the county entirely, which makes the discharge-home leg the long one: a Denver or Aurora hospital back to Castle Rock or Parker is a cross-metro run released mid-afternoon, straight into the northbound-to-southbound turn on I-25.
• All four hospitals and all five Medicare-certified dialysis centres in the county sit in the northern, I-25-corridor half - Lone Tree, Highlands Ranch, Parker and Castle Rock. The county covers 843 square miles: the western third is Pike National Forest and the Rampart Range above Sedalia and Deckers, and the east is open ranch country running out toward Franktown and the Elbert County line. A pickup out there is a long two-lane state-highway drive before any of the trip resembles suburban driving, and there is no closer emergency department to divert to.
• Of the five Medicare-certified dialysis centres in the county, CMS records only DaVita Parker as offering a late shift; Red Hawk, Fresenius Castle Rock, Parker Kidney Center and U.S. Renal Care Lone Tree are day-shift only, so their last chair-off empties into the afternoon and runs back through the I-25/C-470 interchange at Lone Tree, the county's single north-south choke point. The units are small - Red Hawk 8 stations, DaVita Parker 14, Fresenius Castle Rock 15, U.S. Renal Care Lone Tree 16, Parker Kidney Center 17 - so a missed pickup is not easily absorbed into another slot. Note also that UCHealth Highlands Ranch does not open its main, east or garage entrances until 5 a.m.; before that the Emergency Department door is the only way in.
