What we have learned about this county

Estes Park is a care desert by the numbers, and the trip out is a canyon. The CMS registries show zero Medicare-certified dialysis facilities and zero certified nursing homes anywhere in Estes Park - all six Larimer dialysis facilities and all thirteen Larimer nursing homes sit in the Fort Collins, Loveland and Berthoud corridor. So an Estes Park dialysis patient rides down US 34 through the Big Thompson Canyon (or out US 36) three times a week and back up again, and any skilled-nursing placement or SNF discharge for an Estes Valley patient is a down-valley trip by definition, not a local one. Trail Ridge Road, the US 34 route through Rocky Mountain National Park, is closed seasonally, so the canyon and US 36 are the practical ways down.
The county's highest-acuity destination is in Loveland, not Fort Collins, which is counterintuitive if you assume the biggest city holds the biggest hospital. CMS lists exactly five hospitals in Larimer County. UCHealth Medical Center of the Rockies on Rocky Mountain Avenue in Loveland is the Level I trauma centre for northern Colorado; Fort Collins holds Poudre Valley Hospital and Banner Fort Collins Medical Center on Lady Moon Drive; Loveland also holds the Banner North Colorado Medical Center Loveland Campus on Boise Avenue; and UCHealth Estes Valley Medical Center is the county's only critical access hospital. Trauma and tertiary runs therefore point south to Loveland, not to the Fort Collins campuses.
One of the six dialysis sites is not a chair unit, so do not schedule against it. CMS reports the Fresenius Northern Colorado Home Therapy site at 5285 McWhinney Boulevard in Loveland with no in-centre haemodialysis - it is home hemo and peritoneal training only. Only five Larimer facilities actually generate three-times-a-week recurring rides: 1601 Prospect Parkway, 1526 Oakridge Drive and 2107 S. Timberline Road in Fort Collins, and 1453 Denver Avenue and 2940 Ginnala Drive in Loveland.
The Larimer/Weld line runs through the middle of three of this county's towns, and it will bite a service-area map. Windsor and Johnstown straddle the boundary with their town centres on the Weld side - the Generations communities The Windsor and Westwood Villas at 1385 Main Street, Windsor are outside Larimer County entirely. Berthoud also straddles the line, but its core is Larimer: CMS lists Berthoud Care and Rehabilitation at 855 Franklin Avenue as a Larimer County facility. Decide deliberately which side of that line the service area claims rather than naming whole towns.
