Driving here, specifically

What we have learned about this county

  • Every hospital bed and every dialysis chair in Weld County is inside Greeley. The county covers about 3,985 square miles of land - the third largest in Colorado after Las Animas and Moffat - and runs from the I-25 corridor (its west edge sits at roughly 105.06W) east to a genuine 25-mile shared border with Nebraska. Yet both acute-care hospitals (Banner North Colorado Medical Center on 16th Street and UCHealth Greeley Hospital on W. 29th Street) and all three Medicare-certified dialysis units sit within a few miles of each other in that one city. A patient on a standing order anywhere in the eastern two-thirds of the county - Grover, Briggsdale, New Raymer, Stoneham - is driving two-lane highway to Greeley and back three times a week, on roads that drift shut in a plains wind event. Measure the actual mileage per client before quoting; it varies enormously across the county and is much shorter from the southern towns than from the northeast corner.
  • The county's entire late-shift dialysis capacity is one building. CMS lists Fresenius Kidney Care Greeley at 2343 W 27th St (25 stations) as the only Weld County unit flagged for a late shift; North Greeley at 1212 9th St (17 stations) and DaVita Greeley at 2812 W 10th St (12 stations) are both flagged 'No'. Every after-hours dialysis pickup in the county therefore converges on one address off 27th Street, and a run that slips there has no alternative chair to fall back on.
  • Southern Weld looks out of county for hospital care. From Fort Lupton, Firestone, Frederick and Dacono the nearest hospitals are Intermountain Health Platte Valley Hospital (1600 Prairie Center Pkwy, Brighton, ADAMS County) and Longs Peak Hospital (1750 E Ken Pratt Blvd, Longmont, BOULDER County) - both closer than the county seat at Greeley. Straight-line, Firestone is 9.9 miles from Longs Peak but 22.4 miles from Greeley; Fort Lupton is 17.0 miles from Longs Peak and 22.3 from Greeley. US 85 is the working spine between Fort Lupton and Greeley and it is a truck and oil-field road, not a commuter route, while I-25 carries the Carbon Valley toward Longmont and Denver. A Carbon Valley discharge is usually a Weld County pickup with an Adams or Boulder County destination, which changes how the trip is quoted and authorised.
  • Greeley addresses will burn a driver who trusts autocomplete. The city runs numbered Streets and numbered Avenues at right angles, plus Avenue Place and Street Road variants: Banner NCMC is 1801 16th STREET, not 16th Avenue; Westlake Health and Rehab is 1637 29th AVENUE PLACE; The Center at Centerplace is 4356 24th STREET ROAD - all three confirmed against the CMS provider files. Outside the towns the addressing switches again to the Weld County Road grid, which consumer nav apps handle poorly. Confirm the suffix and the WCR number by voice before dispatch, not on arrival.

Named, because "we serve the Denver metro" tells a discharge planner nothing. If your facility is not here, it only means we have not listed it yet.

Hospitals and medical campuses

  • UCHealth Greeley HospitalGreeley · 6767 W. 29th St, Greeley, CO 80634Parking is free and the main patient/visitor lot is on the south side of the building. Entrance hours differ by door: the main hospital entrance is open Mon-Sat 5:30 a.m.-8 p.m., the attached UCHealth Greeley Medical Center entrance closes at 6 p.m. Mon-Sat, and the emergency department entrance on the southwest side is the only 24-hour way in. UCHealth asks that you tell nursing staff if a vehicle will stay in the lot, because security may move cars during winter snow removal - do not park a van and leave it.
  • Banner North Colorado Medical CenterGreeley · 1801 16th St, Greeley, CO 80631Open 24 hours; Banner describes it as a Level II Trauma Center. Banner states that weapon detection systems are in use at this hospital and that all patients and visitors will be screened upon entry into campus buildings - build extra minutes into arrival time for a wheelchair patient with a bag, oxygen or equipment. Visitation hours are 6 a.m.-10 p.m.
  • Banner MD Anderson Cancer Center at North Colorado Medical CenterGreeley · 1801 16th St, Greeley, CO 80631Banner lists this cancer centre at the NCMC street address, so the same campus-entry weapons screening applies. Confirm building and suite with the patient before dispatch.
  • Salud Family Health - Fort Lupton ClinicFort Lupton · 1115 2nd St, Fort Lupton, CO 80621Medical, dental, behavioral health and an in-clinic pharmacy on one site. Medical and behavioral health run 7:30 a.m.-5 p.m. Mon-Fri (dental 8-5, pharmacy 8:30-5); the clinic is closed 8-9 a.m. on the 1st and 3rd Wednesdays of the month, which will break a standing morning pickup twice a month if you do not schedule around it.
  • Salud Family Health - Frederick ClinicFrederick · 5995 Iris Pkwy, Frederick, CO 80504Medical, dental, behavioral health and pharmacy. Medical and behavioral health 7:30 a.m.-5 p.m. Mon-Fri, with the same 8-9 a.m. closure on the 1st and 3rd Wednesdays.
  • Sunrise Community Health - Monfort Family ClinicEvans · 2930 11th Ave, Evans, CO 80620Also Sunrise's administrative address. Open Mon-Thu 7 a.m.-7 p.m., Fri 7:30 a.m.-5:30 p.m., Sat 8:30 a.m.-4 p.m. - unusually wide coverage for the county, and it supports evening and Saturday return trips that most Weld clinics cannot.
  • Sunrise Windsor ClinicWindsor · 1039 Main St, Unit G, Windsor, CO 80550Primary care, pediatrics, family planning and telehealth, 7:30 a.m.-5:30 p.m. Mon-Fri. It is a unit inside a Main Street building, not a standalone campus, so drop-off is street/lot rather than a covered entrance.
